General Description
This position is located at the Little Axe Clinic within the Purchased Referred Care Services Department. This position serves as the liaison between the Absentee Shawnee Tribal Heath, the patients and the third party care providers to whom/what the patients have been referred. The incumbent is under the administrative supervision of the PRC Director.
Responsibilities & Duties
Communicates and coordinates with the Director Case Manager, on various health program entities, physicians and other health care providers to aid in the community of care for tribal members
Communicates with the patients attending physicians when necessary
Participates in opportunities and activities of public relations, promotes a positive professional image in term of attitude and general conduct.
Generate referrals, make appointments, request medical records, generate reports
Specialty Clinic referrals: In-house and Out-of-clinic
Daily medical records review
Utilizes medical knowledge to prioritize referrals, review for medical clarity, and provide patient education
Uses nursing patient model to provide patient centered care for patient needs including anticipation of follow up care, review of medical charts, requesting additional referrals from providers, and updating referrals and orders.
Process Private Insurance referrals
New Patient chart assignment
Work with EHR to establish and maintain paperless referral network
Performs other related duties as assigned.
Education Requirements and Qualifications
Minimum Qualifications:
Graduation from an accredited school of practical nursing conferring eligibility as a Licensed Practical Nurse. Must be currently “Licensed Practical Nurse by the State of Oklahoma.
Must have current basic Life Support Certificate.
Must have a valid Oklahoma Driver’s License.
Must be able to pass a background check.
Must be able to pass a drug test.

Incumbent must be able to perform each essential duty satisfactorily. This position requires a working knowledge and understanding of standards of nursing practice, standards of nursing care, human anatomy, human pathophysiology, basic pharmacology, acute and chronic disease processes, medical terminology and signs and symptoms of complications. Computer and typing skills are required.
Supervision Requirements:
Must be able to perform essential duties with minimal direct supervision. Must possess excellent judgment as related to reporting patient condition of illness to physician or supervisor.
This position is under the direct supervision of the PRC Director/Case Manager and is responsible for directly reporting to same.
