General Description
 The purpose of this position is to serve all of the AST Health System. The AST Health System is an ambulatory health system that currently serves patients with Insurance (Indian and Non-Indian), Absentee Shawnee Tribal Citizens, and Absentee Shawnee Tribal employees and other Native Americans.
Primary function is to work under the supervision of the Models of Care Grant Director. This individual will assist in all phases in the development and implementation of the Dementia Model of Care.
Responsibilities & Duties
Ability to effectively collaborate with team members, including physicians, RNs, and management in the planning and the implementation of Models of Care in accordance with grant requirements.
Dedicated liaison between programs, medical records and clinical staff.
Assist with effective communication and training between clinic personnel, dementia care specialist and grant management.
Monitor compliance with requirements of third party payer, grant management and health system management.
Possess sound organizational skills with the ability to multi-task and with a strong attention to detail.
Skilled in operating phones, computer, software and other IT systems in order to maintain accurate records for program results.
Knowledge of electronic health record systems as well as applications such as Microsoft Word  and Excel.
Assure the continuity of dementia care through delivery of quality patient care and interventions, i.e. Savvy Caregiver in Indian Country and Music & Memory.
Self-motivated and proactive to accomplish goals with a strong sense of accountability for results.
Promote the Absentee Shawnee Tribal Health System philosophy and administrative policies to ensure quality of care.
Ability to deliver Savvy Caregiver in Indian Country training and Music & Memory training to participants of the home and community based caregiver support services, as needed.
Ability to coordinate all aspects for conferences, presentations to include, but not limited to AV needs, room set-ups, food & beverage, meeting materials, etc.
Other duties as assigned by Grant Director.
EDUCATION REQUIREMENTS & QUALIFICATIONS
Education and Qualifications
Certified Medical Assistant, license/certification preferred or a graduate of an accredited school of vocational nursing as a Licensed LPN.
Prefer 2-3 years’ experience as a Certified Medical Assistant or a Licensed Vocational Nurse in a clinical care setting. Preferred Home Health.
Dementia Care Specialist certification (training provided).
Ability to maintain confidentiality and work well with others.
Knowledge, Skills and Abilities:
Excellent computer, clerical and typing skills.
Must be reliable, detailed oriented, capable of multi-tasking with excellent written and verbal communication skills as well as strong professional customer- service interpersonal skills.
Ability to communicate effectively orally and in writing; strong writing skills, ability to relate well with co-workers and the general public.
Ability to adjust to changing conditions.
Must be computer literate required (Windows operating systems – Microsoft Word, Excel).
Must have a valid Oklahoma Driver’s License
Must be able to pass a background check and drug screen.
Nursing skills as defined as generally accepted standards of practice.
Physical Requirements and Working Conditions
Must be able to sit, stand, stoop, bend or kneel for long periods of time.
Sitting or standing or walking for long period of time; occasional bending, squatting, kneeling, stooping; good finger dexterity and feeling; frequent repetitive motions; talking hearing and visual acuity.
