Job Title: Licensed Practical Nurse - Community Health & Prevention
Reports To: Ron McIntosh (Coordinator, Community Directed Grant)
Type of Position: Full time (Hourly)
GENERAL DESCRIPTION:
This position is located in the Department of Community Health & Prevention. The purpose of this position is to provide patient care specifically to the Absentee Shawnee Tribe's diabetic population and strives to promote comprehensive diabetes care for all patients. This position is responsible for promoting best practices, GPRA, and a systematic approach to diabetes care.
DUTIES & RESPONSIBILITES:
- Provide IHS standard of care of all diabetic patients.
- Documents all patient encounters.
- Ensures data is gathered and accurately documented in a timely manner demonstrating the utilization of the appropriate process to promote patient outcomes.
- Participates in activities to promote professional growth and self-development. Attends pertinent in-services, departmental and other meetings as requested by the immediate supervisor.
- Work closely with clinic staff in planning and evaluating patient needs and care.
- Work as a team member within the Department of Community Health & Prevention.
- Seek assistance from team members to confront complex challenges and/or problems.
- Conduct follow-up with patients.
- Ensure return appointments with Primary Care Physician, Dietitian, Fitness Specialist, Nurse, etc. to provide seamless and efficient care across the continuum.
- Assist patients in developing and meeting goals in areas of need and assist in developing treatment plans and assesses them regularly.
- Assess patient's formal and informal support systems; evaluate effectiveness of services provided, treatment and medication.
- Independently manages activities of caseload, while submitting monthly reports.
- Conducts home visits.
- Other duties as assigned.
EDUCATION REQUIREMENTS AND QUALIFICATIONS:
- Must be a LPN in good standing with the Oklahoma Board of Nursing.
- Minimum one year experience working with diabetes patients, diabetes education and case management.
- Requires ability to multi-task, strong interpersonal skills, excellent communication skills, familiarity with diabetes, and culturally sensitive to population and needs.
- Must demonstrate leadership skills, work independently in the absence of supervision, and maintain patient confidentiality.
- Must be computer literate.
- Must possess a valid Oklahoma Drivers License.
- Must be able to pass a criminal background check.
